Customize Command Mini Toolbar?

Is there a way to customize the commands within the CMT that appears upon pre-selection? In the Edit -> Settings dialog there's a way to turn it off and on, but I see no way to customize it. Also, the Tools -> Customize right-click-on-the-toolbar approach does not work. I am having a problem wherein the commands I select in the preselection mini-toolbar do NOT open the command dialog from the task bar. For the life of me I can not find what changed or how to reset the behavior of the CMT buttons.

BTW - if I record myself using the CMT commands, I see that the "UI On Request" behavior is being used instead of "Relative" or "Default". However, if I go into the Customize Toolbar menu and attempt to modify the same commands, the behavior is shown to be set correctly. I do not know where this "UI On Request" setting is coming from, but I really need to change it.
